Step one
Open, complete, and submit the online First Article Order Form.

Or, open, complete, print, and mail (or fax) a copy of the Form in PDF format.

Step two

Email a ZIPPED (preferred format) copy of the master code to firstarticle@siliconserv.com.

Or, you may mail or overnight a copy of the code to the address in Step three below.

Step three

Ship blank devices for the First Articles to:

Step four
Silicon Services will acknowledge receipt of the request and the master code via an EMAIL.
Step five
Our goal is to ship first articles within 24 - 48 hours after receipt of all necessary items.The first article shipped back to you will contain a First Article Aproval form which will require your approval (signature) and be faxed back to 512.490.2290.
